A 58-year-old builder has noticed a bulge in the right groin over 6 months. It appears when he coughs or lifts and disappears when he lies down. On standing, the bulge comes forwards against the examiner's fingers and lies above and medial to the pubic tubercle. After it is reduced, firm pressure just above the midpoint of the inguinal ligament does not prevent it returning when he coughs. The scrotum is normal.
